• Weekend Hours: 2-3 weekend days per month
• Call Requirements: none
• Required Certifications: BLS
• Years of Experience: 2+ preferred
• Float Requirements: Able to float to CRHN and CRHW as needed
• Additional Notes: Prefer experience in IRF setting


Unit Information:
• Number of Beds: 44
• Patient Ratios: Usually carry a 6-7 patient caseload daily
• Equipment:
• EHR: UDS
• Supervisor: • Patient Population e.g. geriatric, adult, peds, etc.: adult IRF


Hiring Profile:
• Must Have Skills/Experience: previous IRF experience highly preferred
• Preferred Skillset/Experience: Adult IRF experience preferred. Not a good fit if only has outpatient, schools, or acute care experience.
• Common Diagnosis/Treatment: Brain injury, stroke, spinal cord, amputee, cardiac, orthopedic
• Personality Fit: Emphasis on teamwork, ok with fast paced environment with short length of stay, working collaboratively with team to DC to home
• Required Education: Graduate of accredited PT program and licensed in IN


First Day Instructions:
• Scrub Color: Any color except navy blue
• Time to Report: 8 am first day. Regular hours after that PT 8-430
• Who to ask for upon arrival: Angie Hill or Michelle McClelland. I will coordinate orientation details with them, so they know what to expect on day one.
• Parking: Anywhere in parking lot, enter through main entrance, check in with receptionist and ask for Angie Hill
