Case Manager

This role focuses on ensuring smooth transitions for patients being discharged. You will actively manage and coordinate the necessary services for patients' post-discharge needs. Effective identification of patients at risk and addressing barriers to discharge is critical. Collaboration with healthcare professionals is essential for optimal patient outcomes. Familiarity with electronic health record systems is beneficial for documentation and care coordination.
