COMPENSATION & BONUS POTENTIAL

$250,000 Base Salary New Residents
$295,000 Base Salary Experienced Physicians
Annual Incentive Bonuses ranging from $45K to $85K+
Incentive payouts can reach up to 28% of your annual base salary

You ll work alongside an experienced multidisciplinary team that includes:

This integrated support model helps reduce administrative burden and allows physicians to focus on delivering high-quality patient care.

PRIMARY RESPONSIBILITIES

Evaluate and treat patients according to established clinical standards
Coordinate patient services including specialty referrals, hospital coordination, SNF coordination, home health, and durable medical equipment
Collaborate with the care team during daily clinical huddles
Maintain accurate and timely medical documentation
Participate in quality improvement initiatives and outcome reviews
Follow established clinical protocols and leadership guidance
Deliver patient-centered care in a value-based setting

REQUIRED QUALIFICATIONS

️ MD or DO from an accredited medical school
️ Active and unrestricted medical license (or willingness to obtain)
️ Board Certified or Board Eligible in:
• Family Medicine
• Internal Medicine
• Geriatric Medicine

️ Active DEA License
️ Medicare Provider Number
️ Medicaid Provider Number
️ Strong interpersonal and communication skills
️ Comfortable working within a team-based care model
️ Basic EMR and computer proficiency

PREFERRED EXPERIENCE

Medicare Advantage experience
Knowledge of HEDIS/STARS metrics
Familiarity with CAHPS & HOS
Experience in underserved or culturally diverse communities
Electronic Medical Record (EMR/EHR) experience
Value-based care experience preferred
