Community Impact Coordinator Full time regular - 40 hours Professional Cherry Hill, NJ, US 3 days ago Requisition ID: 2250 Salary Range: $50,000.00 To $50,000.00 Annually Primary Responsibility Develop and coordinate programs meant for linking the elderly, especially those who are frail, or the disabled residents to the supportive services needed to continue to live independently. Specific Responsibilities Provide information and referrals to residents and families on programs and resources on local, state and federal levels. Interview and screen residents for eligibility for programs and entitlements and assist with application procedures. Assess the needs of residents, including physical, mental, social and financial needs, and develop a plan for delivery of services. Monitor and evaluate services delivery and reassess as necessary. Establish links with agencies and services providers, including the local county division on aging. Contact family members as needed to obtain information, discuss service arrangements, or to encourage family involvement. Plan and implement monthly programs and activities to meet the needs of residents, including establishment of social, educational and recreational programs. Develop volunteer programs and informal support networks. Attend conferences, seminars, and training relevant to the position. Reports that must be submitted in a timely manner. Other duties as assigned. Education & Experience Requirements Bachelor’s degree in psychology, sociology, counseling, or related field Two years of supervisory experience in related field. Solid time-management abilities with the ability to prioritize task.
