Job Opportunity: Registered Nurse

Our organization is looking for a dedicated Registered Nurse to deliver compassionate and skilled in-home healthcare throughout Western Colorado. This position involves a variety of responsibilities aimed at enhancing patient well-being.

• Conduct thorough assessments of patients to evaluate their health status.
• Develop, implement, and regularly update individualized care plans.
• Administer medications and perform necessary treatments.
• Manage wound care, post-surgical recovery, and chronic disease conditions.
• Work collaboratively with a multidisciplinary team to ensure comprehensive care.
• Educate patients and their families about health management and care procedures.
• Document all aspects of patient care accurately and in a timely manner.
• Triage patient needs and coordinate appropriate services.
• Facilitate safe transitions of care to promote patient independence.
• Support individuals in maintaining their quality of life at home.

• Current Registered Nurse license in the state of Colorado.
• Strong assessment and critical thinking skills.
• Excellent communication and interpersonal abilities.
• Experience in home health care is preferred but not mandatory.
• Ability to work collaboratively within a team environment.

This role involves providing care in patients' homes, which may require travel within the Western Colorado region. Flexibility in scheduling may be necessary to accommodate patient needs.
