BAYADA Home Health Care is seeking an experienced and compassionate RN Nurse Case/Clinical Manager to join our Adult Nursing Home Care office in Plymouth, Minnesota. This is an exciting opportunity for a Registered Nurse who is passionate about leadership, mentorship, and delivering care with the highest professional, ethical, and safety standards.

The RN Nurse Case/Clinical Manager serves as a key clinical leader within the office, supporting both client care and staff development. This role blends hands-on clinical involvement with office-based leadership responsibilities. The ideal candidate is a strong nurse leader who can guide field staff, support quality outcomes, and contribute to a positive, mission-driven team culture.

Key responsibilities include providing education and ongoing clinical support to staff, leading and supporting field clinicians, ensuring adherence to BAYADA policies, visiting clients in their homes to supervise field clinicians, maintaining accountability for internal case management activities, evaluating, mentoring, and developing field staff, assisting with in-home client assessments and the care planning process, partnering closely with both clients and office staff to ensure coordinated, responsive care, and monitoring clinical practice, quality assurance, and case oversight across the office.

Qualifications include a current Registered Nurse (RN) license in good standing in the state of Minnesota, minimum of three years of recent, verifiable hands-on experience as an RN or LPN, preferably in home care, at least one year of experience in nursing management or pediatric nursing, BSN or MSN preferred, child abuse clearance where applicable, trach, vent, and tube experience within a home health care setting preferred, along with the ability to teach and train other clinicians, strong clinical assessment and organizational skills, previous supervisory experience with field staff preferred, experience with clinical oversight, quality assurance, and case management preferred, demonstrated ability to work independently while taking on increased or diverse responsibilities, ambition to grow and advance beyond the current role, and strong communication and PC skills.
