Job Title: Social Worker Profession: Social Services Specialty: MSW (Masters of Social Work) Duration: 13 weeks Shift: Day 5x8-Hour Hours per Shift: 08:00 - 16:30 Experience: Minimum of 2 years of acute medical social work experience performing both roles of social work and discharge planning License: CA state license required; pending license accepted if licensed upon start date Certifications: MSW, LCSW either/or MSW Must-Have: - Knowledge of drug and alcohol abuse treatment options - Experience with patients experiencing homelessness and mental health crises - Ability to organize and participate in family meetings - Participation in the development of treatment plans Description: This position involves floating between facilities as needed. You will manage a caseload of up to 25 patients. You will perform care coordination and collaborative care. You will develop plans of care to meet patient and caregiver needs. Discharge planning is a key responsibility of this role. You will participate in care conference activities. Knowledge of advance directive, POLST, and protective services processes is essential. Participation in family meetings is required as part of care delivery. You will work closely with interdisciplinary teams to support patient needs. Weekends are typically required every other weekend. Flexibility to work holidays is necessary, with a commitment to work 3 of the 5 major holidays. The work environment is an acute care facility catering to adults and geriatrics.
