Clinical Coordinator

SUMMARY OF POSITION:

The Clinical Coordinator is a versatile clinical leadership support role responsible for ensuring the continuity of patient care and operational excellence in clinical services. This position functions as a “float leader” providing coverage needed for prioritizing field nurses (direct patient care), and assisting with support in coverage for the Access team, clinical triage, operational nursing leadership support, and social work support. The Clinical Coordinator serves as a resource to clinical leaders by stepping into operational or patient care responsibilities during staffing shortages, vacations, leaves of absences, periods of high census, or other organizational needs. MINIMUM REQUIREMENTS Current RN licensure in New York State, and valid driver’s license with driving record which meets standards outlined in Policy: Qualifications of Hospice Personnel 11-007. Associates degree in Nursing from an accredited program by the National Leave for Nursing. Three to five year’s clinical nursing experience. Previous experience in hospice/home care setting preferred. Bachelor’s degree preferred. Self-directed and able to work with minimal supervision. Has excellent verbal and written communication and organizational skills. Proven ability to work with an interdisciplinary setting and able to work in a team and collaboration with various levels of staff regarding new ideas for education and orientation. Compliant with accepted professional standards and practice. Excellent observation and problem solving skills, math skills, nursing skills per competency checklist, and good nursing judgement and communication skills. Understands hospice philosophy and issues of death/dying and understands principles of pain/symptom management. Ability to deal tactfully with patients and families, healthcare partners and community. Ability to develop education initiatives and create electronic presentations. Strong computer knowledge, including electronic medical records and Mac or Office platforms. Considerable walking or standing. Able to lift, position and/or transfer patients.
