POSITION SUMMARY

Families Forward Resource Center (FFRC) is seeking a part-time Family Resource Navigation Advocate. The Family Resource Navigation Advocate plays a vital role in connecting families and individuals with supportive services and community resources. This position works directly with clients to assess needs, provide referrals, assist with applications, and support families in achieving stability and long-term success. As a Family Resource Navigation Advocate, you’ll be a trusted guide for families seeking support. To be successful in this role, the Family Resource Navigation Advocate needs to be dedicated to community health, Black birthing justice, forward thinking, innovative, reflective, and detail-oriented. FFRC team members are expected to commit to life-long learning to expand expertise, advance programmatic and organizational support to program participants and the community, and engage in quality improvement efforts to optimize support and services. Requirements

ESSENTIAL DUTIES

Serves as the first point of contact for families seeking support and services. Conduct intake assessments to identify needs related to housing, employment, food, healthcare, childcare, and other supports. Provide personalized referrals and warm hand-offs to internal programs and external community partners. Conduct case management and care coordination services as needed based on the needs of the individual and/or family. Complete the Colorado Family Support Assessment 2.0 (CFSA 2.0) as needed in alignment with the needs of the individual and/or family. Use SMART goals to guide and support individuals and families in developing achievable, measurable action plans. Support the Energy Outreach Colorado, including, but not limited to, assisting with application completion, phone calls, and required data entry. Assist clients in completing applications for public benefits and services (e.g., utility assistance). Maintain up-to-date knowledge of available resources and services in the local community. Encourage participation in internal FFRC support, including lactation support, group education, mental wellness support, doula support, and fourth-trimester care as relevant to the program participants. Participate in community outreach events and represent FFRC with professionalism, enthusiasm, and cultural sensitivity. Maintain positive rapport with program participants, community members, organizational partners, and co-workers. Exhibit professional demeanor with program participants, community members, organizational partners, and co-workers. Other duties as assigned. Knowledge, Skills, Abilities and Accountabilities Genuine interest and dedication to Families Forward Resource Center’s mission Genuine interest and dedication to community health and Black Birthing Justice Warm and welcoming Professional attitude and appearance Strong verbal and written communication skills Strong multi-tasking and time management skills Proficiency in Microsoft Office (Outlook, Word, Excel) and Google Products Highly organized, detail-oriented, and able to work independently Experience and ability to identify with individuals and families on their perinatal health journeys with intentional focus on Black and African American community members as those most impacted by maternal and infant health inequities Collaborative team-based approach to support other FFRC programs to ensure effective communication and coordination Ability to adapt to changing circumstances and navigate adversity Participation as a FFRC team player by attending staff meetings and other organizational events Willingness to share knowledge to advance the knowledge and skills of others Understand and commit to follow all policies, procedures, and protocols as set forth in the CNDC/FFRC Employee Handbook Conformance with a drug-free workplace, not under the influence of alcohol and/or other illicit drugs of misuse and abuse A professional, positive, and polite attitude and relationship with other FFRC employees, volunteers, clients, and customers.

MINIMUM QUALIFICATIONS

High school diploma or GED required Associate or bachelor’s degree in social work, Human Services, Psychology, Public Health, or related field preferred At least 1 year of experience in social services, case management, community outreach, or a similar client-facing role Experience working with diverse, low-income, or underserved populations Passion for community advocacy and working with families from diverse cultural backgrounds Other Requirements This is an in-person position—candidates must be comfortable working onsite and interacting with clients face-to-face Reliable transportation and a valid driver’s license Bilingual (English/Spanish or another language) is required About the Company Families Forward Resource Center (FFRC) is a full-service family resource center providing supportive services and programs to northeast Denver and north Aurora metro areas.
