Responsibilities Coordinate the home health referral and intake process, ensuring timely, appropriate, and compliant patient admissions. Monitor, analyze, and report quality performance data to support regulatory compliance, quality improvement, and patient outcomes. Conduct quality assurance reviews of clinical documentation and provide feedback, education, and support to staff. Serve as the agency's EMR Super User, assisting with training, troubleshooting, workflow optimization, and software updates. Collaborate with providers, referral sources, patients, caregivers, and agency staff to support effective care coordination and operational efficiency. Knowledge & Experience Current Registered Nurse (RN) license required. Current BLS certification required. Valid driver's license and vehicle liability insurance in accordance with Alaska state law. Two years of recent clinical nursing experience required. Home health experience with Medicare patients and OASIS documentation preferred. Strong clinical knowledge, critical thinking, and ability to exercise independent judgment. Demonstrated computer proficiency, including Microsoft Word, Excel, Outlook, and electronic medical record systems. Physical ability to sit, walk, stoop, travel between locations, and perform light-to-moderate physical activities associated with patient care and office work. The South Peninsula Hospital is a combined 22-bed Acute Care Critical Access Hospital and 28-bed Long Term Care facility located at the southern end of the Kenai Peninsula in South-Central Alaska. This position works primarily in the Home Health office coordinating patient admissions, quality improvement activities, and documentation review, while also supporting field operations as needed. The role requires collaboration with patients, caregivers, providers, community partners, and clinical staff to ensure high-quality home health services. Job Hazard: Potential hazards include vehicle travel, falls, paper cuts, eye strain from computer work, exposure to infectious diseases, and other risks commonly encountered in healthcare environments.
