Senior Services Case Manager - Community Navigator

ACC Senior Services

ACC Senior Services is seeking a dedicated Caseworker in Sacramento to provide meal delivery services to frail, at-risk older adults. This full-time position involves screening referrals, maintaining a participant caseload, and coordinating community resources to enhance the well-being of clients. Candidates should have a High School Diploma and experience in Social Work or a related field. Proficiency in computer skills and a valid California Driver’s License are required.
