Occupational Therapist (OT) - SNF / LTC - Contract

Job Summary A great SNF / LTC is looking for an experienced or newly graduated Occupational Therapist / OT to join their growing team for a contract assignment! Responsibilities Provide direct patient care to assess medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Establish and administer a treatment program with specific goals determined according to the patient’s capacity and tolerance under the direction of the Physician. Set realistic and achievable goals for patients. Document and record the patient’s condition. Educate patients and families in an appropriate occupational therapy method. Qualifications Graduated from an accredited school. Current state license as an Occupational Therapist / OT or actively in the process of applying for licensure. Current CPR certification. Provide skilled occupational therapy services/interventions in accordance with physician orders. Assess patient needs, plan for, evaluate and modify care to meet goals of occupational therapy interventions. Collaborate with all disciplines to plan and evaluate team goals for each patient.
