LPN Field Clinical Support- Bilingual Chinese Speaking

Overview

Provides licensed practical nursing support to the Medical Care at Home (MCAH) clinical team during in-house and virtually supported preventive assessments. The LPN, Field Clinical Support is physically present with the patient and works under the direction of the Nurse Practitioner (NP), collecting and communicating objective clinical information, completing structured intake activities, supporting Care GAP closure, performing ordered procedures and testing within LPN scope and demonstrated competency, and reinforcing patient education. The NP maintains responsibility for the clinical assessment, interpretation of findings, diagnoses, Assessment and Plan, and final encounter documentation.

• Prepares patients for MCAH preventive and comprehensive assessment visits and completes structured intake activities, including collection of medical history, medication information, vital signs, functional information, and other objective clinical data for NP review.

• Works under the direction of the NP during virtual encounters and communicates objective findings, changes in patient condition, and other clinical observations requiring NP review. 

• Supports medication reconciliation by comparing available medication information with patient-reported use, identifying discrepancies, and communicating findings to the NP for assessment and follow-up.

• Performs ordered clinical procedures, screenings, specimen collection, point-of-care testing, and other nursing activities that are within LPN scope, organizational policy, and demonstrated competency.

• Performs phlebotomy and ensures specimens are collected, labeled, handled, stored, and transported according to established protocols when applicable. 

• Supports completion of identified Care GAPs by reviewing assigned preventive needs, facilitating ordered testing, documenting completion, and communicating barriers or outstanding needs to the NP and appropriate team members.

• Provides NP-directed examination support and reports objective clinical findings in real time during the virtual encounter. The LPN does not independently diagnose, triage, or establish the clinical Assessment and Plan.

• Utilizes TytoCare or other approved remote examination technology when available and clinically indicated to facilitate additional NP examination. When further examination cannot be appropriately completed during the visit, supports coordination of follow-up as directed by the NP.

• Recognizes and promptly reports abnormal findings or changes in patient condition to the NP. Follows established escalation and emergency protocols, including activation of emergency services when directed or clinically required by protocol.

• Provides and reinforces patient and family health education, preventive health information, specimen collection instructions, medication-related teaching, and NP recommendations within LPN scope and organizational policy. 

• Accurately documents patient information, nursing activities, clinical observations, procedures, testing, and communication in the electronic medical record (EMR).

• Ensures documentation is complete, timely, accurately identified, and includes required signatures, dates, orders, and supporting information. 

• Travels to patient homes, community settings, or care facilities and adapts to varied field environments while following safety and infection prevention standards.

• Transports, operates, cleans, and maintains required clinical equipment and supplies in accordance with manufacturer guidance and organizational policy.

• Participates in interdisciplinary team meetings, case conferences, clinical supervision, education, and competency activities to support continuity and quality of services.

• Communicates with Care Managers, Primary Care Providers, scheduling staff, and other members of the care team to support referrals, follow-up, and completion of the NP plan. 

• Supports daily operational needs of the VNS Health-affiliated Medical Care at Home practice and contributes to quality, productivity, Care GAP closure, and performance goals. • Participates in special projects and performs other duties as assigned.

• The LPN collects, documents, and communicates clinical information within LPN scope and competency; the NP performs the clinical assessment and interprets the findings.

• The NP is responsible for diagnoses, clinical decision-making, the Assessment and Plan, and determining whether additional examination or escalation is required.

• If additional examination is needed beyond what can be appropriately completed during the LPN-assisted virtual visit, the NP determines the next step. This may include a TytoCare-facilitated follow-up, an in-person NP visit, PCP/specialist follow-up, urgent evaluation, or emergency services based on clinical urgency.

• The LPN does not independently perform medical diagnosis, independent triage, or independently establish the patient's plan of care.

• Participates in special projects and performs other duties as assigned.

Qualifications

Licenses and Certifications:
License and current registration to practice as an LPN in New York State required. Phlebotomy competency/certification preferred.

Valid NYS Drivers License or ability to travel throughout assigned service areas, consistent with role requirements required.

Education:
Graduate of an approved practical nursing program required

Work Experience:
One year of LPN experience in a medical practice, ambulatory care, home care, community health, or similar setting. required.

Experience supporting virtual care, preventive assessments, Care GAP closure, home-based services, phlebotomy, or point-of-care testing preferred.

Strong interpersonal, organizational, documentation, and multitasking skills; proficiency with EMR systems and Microsoft Office/Teams preferred 

Pay Range

USD $28.09 - USD $35.08 /Hr.
