Minimum Requirements
Associate, Bachelors, Masters, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
Active Alabama or multistate Physical Therapist license required prior to start of assignment.
Valid drivers license required.
Current BLS/CPR certification from AHA or ARC required.
Home Health experience preferred.
Ability to perform patient care and physical therapy duties in home health settings.
Professional Physical Therapy certification as required by role.

Assignment Details
Facility Type: Home Health.
Shift Name: Evenings.
Shift Details: Evenings 2000-0400.
Coverage Areas: Eufaula and Dothan, AL.
Weekly Hours Guarantee: 40 hours.
Productivity Expectation: 30 points.
Weekend Requirement: Occasional weekend work.
Mileage reimbursed between patient visits from first patient to last patient.
Dress Code: Navy scrubs and closed-toe shoes.
