Key Qualifications:

• Active and unrestricted RN license
• Minimum of one year of recent Home Health nursing experience
• Proficiency in home health documentation systems (e.g., OASIS)
• Strong assessment, critical thinking, and clinical skills
• Reliable transportation and willingness to travel between patient locations
• Ability to work independently and flexibly adapt to changing patient needs
• Excellent communication and interpersonal abilities

Responsibilities:

• Conduct patient assessments, develop individualized care plans, and monitor progress
• Administer medications, wound care, and other prescribed treatments
• Educate patients and families on managing conditions and medications at home
• Coordinate care with physicians, therapists, and the broader interdisciplinary team
• Accurately document each visit and update care plans as needed
• Recognize and respond to changes in patient condition, escalating care when necessary
• Provide compassionate support to enhance patient comfort and outcomes

$1,573 - $1,664/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
