Catholic Charities is the official domestic relief agency of the Archdiocese of Portland in Oregon.POSITION SUMMARY:
The Adult PSH Case Manager is responsible for the overall delivery of services and implementation of individualized case plans for the families in the Permanent Supportive Housing (PSH) program, including crisis intervention counseling, harm reduction, housing stabilization, job search, and maintaining permanent housing. They will also lead classes, groups, and events to an identified caseload of clients. The position requires an experienced Case Manager to work with families challenged by years of homelessness, trauma, substance use, and behavioral and physical health issues. The Adult PSH Case Manager will use proactive prevention, best-practice approaches, and collaborative interventions to support residents to thrive and retain their housing. The position works closely and coordinates with onsite Property Management staff, other Resident Services staff, and with external partnerships.PRINCIPLE DUTIES AND RESPONSIBILITIES:
Provide evidence-based case management services to 15-20 families residing at Good Shepherd Village with an emphasis on the adult members of the household. Complete initial intake and assessments of families to assess their needs in order to implement or connect them with services or treatment. Assist residents in applying for and obtaining long term stable sources of income including but not limited to mainstream benefits, as well as OHP, TANF, SNAP, Head Start and other programs for which they are eligible. Use Housing First and harm reduction interventions and methodologies to provide resident centered services that support them in retaining their housing. Attend at least quarterly training to increase knowledge of effectively meeting residents’ needs (co-occurring disorders, harm reduction, cultural responsiveness, etc.). Work closely with other agency programs to maximize residents’ outcomes and program goals. Provide appropriate resources, tools, and counseling to assist residents in achieving their case plan goals; making referrals for services and following up to ensure the referrals were completed. Maintain work schedule as required by program needs with adequate flexibility. Document and maintain up to date information on services provided to residents within 24 hours of resident meeting or any case development. If clients exit Good Shepherd, ensure that 85% transition to permanent housing. Ensure 55% of residents gain earned income/increase income. Coordinate on-site adult enrichment classes to meet the needs of residents and support residents to increase self-confidence and self-sufficiency, including Rent Well as needed. Arrange and facilitate monthly resident’s meetings/family events. Conduct weekly home visits and/or office appointments with residents to review progress toward goals and self-sufficiency. Using best practices, develop case plans and goals with assigned clients, focusing on five main domains: housing stability, income, social connection, health and emotional resilience and thriving children. Work collaboratively with identified, and to be identified, community partners to enhance the services that we provide. Serve as a primary liaison between agency and community resource agencies that assist clients. Participate in community networks focused on families with children, including Coordinated Access for Families and case conferencing. Remain updated on coordinated access assessments for families and conduct assessments of eligible families referred to the Family Services team. Coordinate with on-site Resident Services and property management. Meet standards for documentation and perform data entry related to client care, client assistance, community referrals, and quality indicators in the electronic record according to established guidelines. Participate in trainings and meetings assigned by a member of management, their supervisor, and/or Human Resources. Adherence to Catholic Charities policies and procedures. Perform other duties and responsibilities as assigned.QUALIFICATIONS:
Bachelor’s degree in human services, social work, early childhood education or equivalent lived and or worked experience required. Master’s degree in Social Work or in related field preferred, with 3 years’ experience working with diverse populations and families in crisis. 1 year case management required. 3 + years of case management experience within social services, homeless services and/or a PSH setting, preferred. Experience working with and demonstrated understanding of unique challenges for vulnerable families including but not limited to those experiencing homelessness, trauma, mental illness, addictions, and chronic medical needs. Ability to respond to crisis situations. Ability to become certified as a Rent Well trainer within 6 months. Excellent written and oral communication skills; Bilingual/bicultural preferred. Ability to identify, implement, and coordinate the services and supports a client’s identified need. Knowledge and / or experience with community resources serving families in Clackamas County including: WIC, SNAP, TANF, Head Start, and the domestic violence services. Ability to lead groups and classes, such as Rent Well, as well as understanding of adult learning. Ability to use the following tools and approaches: Assertive Engagement, motivational interviewing, person-centered case management, strengths-based practices and trauma- informed care. Demonstrated competency working with people from diverse cultures. Ability to assess and treat clients in a culturally competent manner. Ability to effectively work with standard office programs including Word, Excel, Outlook, Microsoft Teams and PowerPoint. Demonstrate judgment and discretion in dealing with confidential matters. Commitment to Catholic Charities mission to work in partnership with vulnerable populations to achieve lasting solutions to poverty and injustice. Must be able to organize and prioritize work, be proactive, take initiative, follow through, and simultaneously manage multiple priorities to ensure goals are met in a timely manner. High attention to detail required. Willingness to learn new skills and take on new responsibilities. Strong analytical and strategic problem-solving skills. Ability to work well in team setting, as well as independently; be flexible and adapt well to different dynamics in a fast-paced work environment. Ability to work a flexible schedule, which could include some evenings and weekends. Ability to lift 40 lbs. Travel between sites in Portland required. Must have driver’s license, access to vehicle and ability to obtain automobile insurance at levels required by agency.COMPENSATION AND BENEFITS:
