Position Title: Community Resource Coordinator

Location: Albany, GA – Remote/Field-Based - 90% Traveling

Duration: 12+ months (Possible to Extension OR convert to FTE)

Pay Rate: $25/hr

Shift :

9-6 M-T and Fri 8:30-5:30pm

Overtime may be required depending on the assignment. Day-to-day travel can range from 5 minutes to 2 hours, with travel time varying by assignment. Mileage is compensated. The candidate may also need to travel to community events in Savannah, Augusta, or Macon approximately twice a year.

 

PRIMARY PURPOSE:

The Community Resource Coordinator provides support to health plan members by connecting them with appropriate community, social, and healthcare resources. This is a member-facing, field-based position that focuses heavily on community outreach, home visits, resource coordination, and addressing Social Determinants of Health (SDOH) barriers.

The ideal candidate will be empathetic, community-focused, energetic, adaptable, and comfortable working independently while collaborating with nurses, social workers, healthcare professionals, and community organizations.

Key Responsibilities

• Connect members with appropriate community, healthcare, and social service resources in a cost-effective manner.
• Conduct telephonic and in-person outreach to locate hard-to-reach individuals and families.
• Conduct home visits with high-risk members to complete non-medical assessments.
• Identify barriers affecting members’ ability to access care, including transportation, employment, home safety, and other SDOH needs.
• Assess community and environmental resources and connect members with appropriate services.
• Provide informal guidance, education, and support to members regarding available community resources.
• Assist nurses, social workers, and clinical teams with outreach, community education, and member support activities.
• Respond to member inquiries regarding community resources and available services.
• Prepare and support member-related correspondence and educational materials.
• Maintain accurate documentation of community resources and member interactions in accordance with health plan requirements.
• Gather information through assessments, interviews, and outreach activities to support healthcare providers and internal teams.
• Build relationships with community partners and organizations to help address member needs.
• Participate in and coordinate community outreach events throughout the designated region and, when required, across the Georgia market.
• Make visits to individual homes and community organizations as needed.
• Maintain knowledge of Social Determinants of Health and available community resources.
• Follow all organizational policies, procedures, standards, and compliance requirements.
• Perform other duties as assigned.

Candidates should be comfortable:

• Driving extensively throughout Georgia.
• Visiting members in their homes.
• Working with high-risk and hard-to-reach populations.
• Identifying social and environmental barriers to healthcare.
• Connecting members with community resources.
• Working independently while collaborating with clinical and community teams.
