Job Description
Title: RN - Home Health

Schedule: Monday to Friday with weekend rotation approximately every 4th week

Assignment Details:
- Guaranteed Hours: 36
- Contract Length: 12 weeks

Requirements:
- Active RN license

Roles & Responsibilities:
- Provide individualized, goal-oriented nursing care following the nursing process in accordance with physician orders.
- Serve as a Case Manager when acting as the lead discipline in care coordination.
- Collaborate with interdisciplinary teams to enhance patient care.
- Maintain accurate documentation of patient care and treatment plans.
- Educate patients and families about health management and support services available.
- Address patient needs and ensure compliance with healthcare regulations.
