Care Manager - Integrated Health & Behavioral Services
FLACRA
FLACRA Care Manager role provides comprehensive management and coordination for individuals with co‑occurring chronic health and behavioral health needs. A reliable vehicle is required for this position, with collaboration across medical, behavioral health, and social services to ensure appropriate care and support.
Key duties include linking clients to housing, employment, transportation, medical and community resources, and coordinating with FLACRA clinic providers using Telehealth and EHR
