This position offers a remote work setting, allowing for flexibility while maintaining a strong connection to our clients and their families. Your schedule will primarily adhere to a Monday through Friday routine from 8:00 AM to 4:30 PM, but some evening or weekend hours may be necessary based on client needs.

Key Responsibilities\n

• Conduct comprehensive assessments of clients' psychosocial needs through interviews, observations, and standardized evaluation tools to ensure a holistic understanding of their requirements.

• Collaborate closely with seniors and their families to design and implement individualized care plans that prioritize their goals and preferences, ensuring an inclusive approach to care.

• Provide compassionate counseling and emotional support to clients, assisting them in managing life transitions such as health changes, loss, or chronic illness.

• Advocate for clients by identifying their rights and connecting them with community resources, including housing assistance, transportation, and healthcare services.

• Regularly review and adjust care plans based on ongoing assessments and progress, fostering a dynamic support system for your clients.

Your primary focus will be on seniors, many of whom may be experiencing complex life changes and require specialized support in areas such as mental health management, coping with physical health challenges, and navigating family dynamics. You will utilize your expertise to empower clients, helping them to build resilience and maintain their independence while ensuring access to necessary services.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• Possession of a current and valid state licensure as a Licensed Clinical Social Worker (LCSW).

• A minimum of two years of experience in a relevant social work environment, ideally with a focus on geriatric populations or community health settings.
