Hospice Case Manager

We're seeking a skilled and empathetic Full Time Hospice Case Manager to join our dynamic team in the Flint and surronding areas. This position offers a rewarding opportunity to make a significant impact in the lives of those we serve. Key Responsibilities: Conduct initial and ongoing assessments of patients’ needs to develop comprehensive care plans. Provide emotional support and education to patients and families about end-of-life care and self-care techniques. Collaborate with the interdisciplinary team to ensure high-quality care and regular updates on patient progress. Supervise and mentor nursing staff and paraprofessionals in delivering exceptional care. Maintain accurate clinical documentation and participate in performance improvement initiatives. Qualifications: Current Michigan Registered Nurse (RN) License in good standing. Minimum of 1 year experience in hospice or palliative care. Strong assessment and critical thinking skills. Excellent communication and interpersonal skills. Ability to work collaboratively in a team environment. Empathetic and compassionate approach with patients and families. Willingness to participate in on‑call duties as needed.
