Clinical Coordination ManagerThe Clinical Coordination Manager is a key member of the Clinical Leadership Team, responsible for overseeing care coordination, clinical quality, and continuity of care at our Woodinville pediatric home. This role ensures that medically fragile children receive safe, physician-directed, and holistic care in a structured, team-based environment.This position reports directly to the Administrative Manager and works in close partnership to ensure operational and clinical alignment.Core ResponsibilitiesOversee and coordinate individualized plans of care in alignment with physician ordersEnsure quality, safety, and continuity of care across all shiftsProvide regular clinical updates to staff and leadershipCoordinate and schedule client appointmentsServe as liaison between physicians, therapists, vendors, families, and Ashley House team membersConduct regular chart audits to ensure accurate documentation of medications and treatment ordersTrain and support staff in Electronic Medical Records (EMR) systemsAssist in interviewing, hiring, onboarding, and mentoring clinical staffProvide 1:1 training and clinical competency oversightAddress staff performance concerns and implement corrective action in partnership with leadershipParticipate in rotating clinical on-call coverage (every other week)Ensure house safety, clinical compliance, and cleanliness standards are upheldThis role requires strong clinical judgment, sound documentation practices, and the ability to lead with both authority and compassion.About Country HomeFormerly Children's Country Home, this eight-bed nonprofit has served medically fragile children since 1997.
