Hospice Liaison

Job Description

Valley Oaks Hospice is currently seeking to hire a professional healthcare liaison/marketer. Serving Orange County for since 2017, we are a company committed to provide exceptional service to all the patients and families we serve and are looking to add someone that share the same vision.

The ideal candidate would have extensive experience in marketing in the healthcare field - preferably hospice or home health. Having strong, existing relationships with possible referral sources is a huge plus. In addition, the ideal candidate would be able to:

• Establish relationships with referral sources such as physicians, hospitals, long-term care facilities and assisted living facilities.
• Apprises the Hospice Director or Sales Manager on a predetermined basis of scheduled presentations.
• Educates referral sources on the components of hospice care services.
• Serves as a liaison between physicians, hospitals, long-term care facilities, assisted living facilities and all other referral sources to identify healthcare needs.
• Communicates frequently with each referral source to ensure that the expectations of the referral source and the needs of the patients are being met.
• Serves as a liaison between hospital and nursing facility discharge planners by visiting patients, as requested, to ensure a smooth transition to the patient’s home.
