ATHENA places a high value on customer service, accountability and getting the job done. POSITION Summary: We are currently recruiting a Care Manager for the PROMISE Program. This full-time position pays $23.50 an hour and is in Newark, DE. ESSENTIAL DUTIES AND Responsibilities Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Conduct comprehensive assessments of clients' strengths, needs, and goals. Develop, implement, and monitor individualized care plans in collaboration with clients, families, and service providers. Provide referrals and linkages to behavioral health, medical, housing, employment, and social service resources. Coordinate with treatment teams to ensure continuity of care and successful outcomes. Maintain accurate and timely documentation in compliance with agency and state regulations. Monitor client progress and adjust service plans as needed. Educate clients and families about available supports, treatment options, and community resources. Participate in interdisciplinary team meetings and case conferences. Respond to crises and help connect clients with emergency services when necessary. Additional duties as assigned. Required Skills/Abilities Excellent organizational skills and attention to detail. Strong interpersonal skills and the ability to build trust with diverse populations. Ability to work independently and manage multiple cases effectively. Excellent verbal, written, and listening communication skills. Knowledge of behavioral health systems, community resources, and case management best practices. Empathy, patience, and professional judgment when addressing client needs. Education and Experience Bachelor's degree in Social Work, Psychology, Human Services, or a related field, required. Two (2) years of professional experience in case management, behavioral health, or human services, preferred. Valid driver's license and reliable transportation, required. Physical Requirements Prolonged periods of sitting at a desk and working on a computer. Standard hours of work Monday through Friday, 8:30 am - 5:00 pm, however evening and weekend hours may be required. Travel within the community or between sites may be required.
