Responsibilities Coordinate medical records for clinical nutrition. Ensure medical records reflect nutritional status of the individuals and meet regulations. Maintain annual completion of assessments using the Nutrition Care Process. Attend ISP meetings, as well as entrance and exit meetings. Complete nutritional assessments, care plans, progress notes and discharge summaries. Make recommendations to the treatment team for the maintenance and/or improvement of nutritional status. Recommend changes to meal plans to address nutritional concerns. Process diet changes and provide the necessary training to the individual and staff concerning the change. Observe individuals at mealtime, which may involve travel to a variety of service locations. Provide nutrition care instructions to individuals, families, family care providers, food and nutrition staff and other health‑care providers in residential and Article 16 clinic settings. Work as part of an interdisciplinary team to determine the nutritional needs of the individual. Schedule and conduct nutrition in‑service training/education for house staff; maintain attendance records and records of topics covered. Implement plans for corrective action. Ensure OPWDD regulations are adhered to and overall health and safety of the individuals is maintained. Use a computer proficiently and have knowledge and use of an EHR system. May be assigned to or asked to assist in other assignments. Qualifications Promotion: Current registration with the Commission on Dietetic Registration (CDR) as a Registered Dietitian (RD) or New York State Education Department (NYSED) certification in dietetics‑nutrition and one year of service as a Dietitian 1. Open Competitive: Current registration with CDR as an RD or NYSED certification in dietetics‑nutrition and completion of a minimum of 400 hours of supervised dietetics practice under a registered dietitian. Maintain current certification in New York State as a Certified Dietitian/Nutritionist. Transfer: Candidates must have one year of permanent competitive or 55‑b/c status in the posted title and be eligible for a lateral transfer; or be eligible to transfer via Section 70.1, 70.4, or 52.6 of Civil Service Law. The transfer cannot be a second consecutive transfer resulting in an advancement of more than two salary grades. Benefits Annual salary: Grade 18 ($66,951) plus Downstate adjustment ($4,000) for a total of $85,138. Paid holidays: 13 holidays per year. Vacation leave: Initial 13 days. Personal leave: 5 days per year. Sick leave: 13 days per year (may carry over). Professional leave: Up to 3 days per year.
