Key Responsibilities\n

• Conduct comprehensive psychosocial assessments through interviews, observations, and the review of medical and educational records to evaluate the emotional, social, and environmental factors affecting a patient's recovery process.

• Provide counseling and emotional support to patients and their families, addressing adjustment issues such as coping with the aftermath of injury or illness.

• Collaborate with a team of rehabilitation professionals—including physicians, occupational and physical therapists—to design and implement personalized treatment plans that are tailored to meet the psychosocial needs of each patient.

• Assist patients in navigating community resources, including support groups, vocational rehabilitation services, and housing assistance, ensuring they have access to necessary support systems.

• Educate patients and families regarding community resources and post-rehabilitation services, helping them understand the continuum of care.

• Participate actively in discharge planning, ensuring smooth transitions to home or alternative care settings.

Your primary focus will be on individuals in a school environment who are facing emotional and social challenges due to various health conditions, disabilities, or life situations. Qualifications\n

Compensation and Benefits\n

The annual salary for this role ranges from $75,000 to $100,000, commensurate with experience and qualifications.
