8:00 AM - 4:30 PM, Sunday to Thursday

Assignment Details

Guaranteed Hours: 36.00 Contract Weeks: 13

Requirements

• Bachelor's Degree in Nursing (BSN): Preferred
• Registered Nurse (RN) licensure in the state of practice: Required
• Basic Life Support (BLS) certification from approved vendor: Required
• Experience in a care management role: Preferred
• Case management certification: Preferred
• At least two years recent experience in field: Required

Responsibilities

• Lead the coordination of patient care with the healthcare team, ensuring the appropriateness and timeliness of care.
• Monitor patient transitions through the care continuum from pre-admission to post-discharge.
• Act as a liaison among patients, families, and healthcare providers, optimizing patient outcomes.
• Collaborate on complex cases, including frequent emergency department visitors and specific patient populations.
• Coordinate transfers to other facilities, ensuring continuity of care and required patient information.
• Collect and manage data to support regulatory and accreditation compliance.
