Talbert House
Talbert House is seeking a Case Manager in Mount Orab, OH to oversee full client care cycles. You will assess, plan, implement, monitor, and evaluate services to meet health and human services needs. Under minimal supervision, you will coordinate care, maintain caseloads, develop service plans, and document activities in accordance with agency policies.
