Work Environment

This role is set in an onsite home care facility where you will be providing in-person therapy for our patients in their homes or community settings. Your work schedule will follow a standard Monday to Friday routine, from 9 AM to 5 PM, allowing you to maintain a healthy work-life balance. We emphasize a collaborative approach to patient care, providing our therapists with the autonomy to develop personalized treatment plans while fostering communication with interdisciplinary teams.

Key Responsibilities

• Conduct thorough evaluations of patients' physical abilities, establishing personalized treatment plans that address their specific needs and goals.
• Deliver skilled and compassionate therapy services, employing therapeutic exercises, manual therapy techniques, and various modalities suited to geriatric care.
• Regularly monitor and document patient progress, adjusting treatment approaches as necessary to promote optimal health outcomes.
• Work closely with healthcare professionals, including physicians, occupational therapists, and speech therapists, to ensure a comprehensive and holistic approach to each patient's care.
• Educate patients and their families on therapeutic techniques, developing home exercise programs, and implementing injury prevention strategies.
• Implement fall prevention programs and necessary safety measures tailored for the assisted living or home care environments to mitigate risks for our aging population.
• Stay current with advancements in physical therapy through continuing education to enhance your therapeutic skills and apply the latest techniques in your practice.
• Maintain high ethical and professional standards, ensuring patient confidentiality while delivering the highest level of care.

Patient Population / Client Focus

You will primarily work with geriatric patients who may face a variety of challenges including mobility issues, chronic pain, and post-operative rehabilitation needs. Our focus is on empowering these individuals to achieve self-sufficiency and improve their quality of life in a familiar setting, promoting their ability to age in place.

Qualifications

• A graduate of an accredited college or university with a Bachelor's, Master's, or Doctorate degree in Physical Therapy.
• Active State licensure as a Physical Therapist is required.
• Strong interpersonal and communication skills, enabling effective interactions with patients, families, and other healthcare professionals.
• Proficiency with electronic medical records (EMRs) and documentation systems relevant to therapy practices.
• A strong foundation in evidence-based practices and the ability to apply innovative therapeutic techniques.
• Dedication to providing patient-centered care that promotes patient independence.
• Demonstrated ability to work effectively both independently and as part of a multidisciplinary healthcare team.

Preferred Experience

• Prior experience in geriatric therapy is preferred but new graduates are encouraged to apply and can gain valuable experience in the field.
• Familiarity with home care settings and interdisciplinary collaboration is advantageous.

Compensation and Benefits

We offer a competitive annual salary ranging from $100,000 to $120,000, commensurate with experience and qualifications.
