MDS COORDINATOR Per‑Diem MDS Coordinator – Basking Ridge, NJ 07920 Responsibilities of MDS Coordinator: Must have experience with MDS/PDPM process for Sub‑Acute and LTC residents Must have experience operating Point Click Care for assessments and EMR Collaborate with facility’s staff to select the correct assessment of our residents Must have good communication skills in order to work effectively and keep accurate, detailed records Ensure all MDS are in compliance with State and Federal regulatory requirements Participate in developing, updating, reviewing, and completing individualized Care Plans Schedule: 8 hour shift Day shift Education: Bachelor’s (Preferred) Experience: MDS: 1 year minimum (Required) License/Certification: RN (Preferred) RAC‑Certified (Preferred) Benefits: 401(k) Work Location: Basking Ridge, NJ 07920 In Person/Off Site EOE/AA: FellowshipLIFE is an equal opportunity employer.
