Case Manager — Housing & Life Skills for Youth

Lutheran Social Service of Minnesota in Minneapolis is seeking a Case Manager to provide housing transition and independent living support to individuals in care or recently exited programs. You will coordinate with county social workers, foster parents, probation and community agencies to develop plans, facilitate life skills and parenting groups, lead bi-weekly program meetings, and collect reports for program management. #J-18808-Ljbffr Lutheran Social Service of Minnesota
