Community Health Assistant

HarmonyCares, a physician-led home-based primary and palliative care organization, is seeking a Community Health Assistant (CHA) to support medically complex, home-limited patients. In this role, you’ll assist with care coordination, conduct in-home and telehealth visit support, gather vital patient information, and reinforce care plans under clinical supervision. Working within an interdisciplinary team, you’ll help reduce avoidable hospitalizations and improve outcomes.
