RN Case Manager
Las Palmas Medical Center
Las Palmas Medical Center in El Paso, TX seeks a Registered Nurse Case Manager to support our ER team. In this role, you will coordinate care for high-acuity patients, conduct assessments, develop and implement care plans, and ensure safe, timely transitions through the continuum of care. You will collaborate closely with physicians, social workers, and ancillary staff to optimize outcomes, reduce readmissions, and advocate for patient and family needs. We value strong clinical judgment, calm performance in intense situations, excellent communication, and a passion for compassionate, community-focused care.
