Registered Nurse Case Manager

Del Sol Medical Center in El Paso seeks a Registered Nurse Case Manager to coordinate patient care across the continuum in a busy, full‑service acute care hospital. In this role, you will assess clinical and psychosocial needs, develop and implement individualized care and discharge plans, and collaborate with physicians, nurses, and community providers to ensure safe, cost‑effective transitions. You’ll advocate for patients, support families, perform utilization review, and document in the EHR while contributing to a compassionate, patient‑first, team‑oriented culture with opportunities for growth and ongoing training.
