Key Responsibilities\n

• Conduct comprehensive evaluations to assess each patient's unique needs and devise tailored treatment plans.

• Create and share actionable plans with supervisors that outline patient appointments, home visits, and essential observation sessions.

• Organize home visits to conduct in-home assessments, perform chart reviews, and analyze data to ensure aligned care with clients' needs.

• Participate in inter-departmental meetings and collaborative care events to ensure cohesive treatment strategies are implemented.

• Compile monthly and quarterly progress reports that detail patient advancements and challenges, ensuring continued alignment with treatment goals.

• Lead training sessions for staff and allied health team members on program execution and documentation best practices.
\n
• Conduct follow-up visits to gauge ongoing patient needs and ensure consistent support throughout their treatment journey.

• Maintain organized records to guarantee all pertinent patient information is accessible to authorized personnel.

• Serve as a liaison between the general community and the mental health sector, promoting awareness and resources to help individuals achieve their health goals.

Qualifications\n
