Key Responsibilities\n

• Conduct comprehensive assessments and evaluations to determine the unique mental health needs of each patient, establishing tailored treatment plans.

• Document and track therapeutic interventions and outcomes, sharing insights and plans with supervisors through regular meetings and reports.

• Organize and conduct home visits when necessary to ensure thorough assessments, complete chart reviews, and support personalized care plans.

• Actively participate in Interdisciplinary Team Meetings to collaborate on patient care strategies and share knowledge across disciplines.

• Complete monthly and quarterly progress reports while ensuring that all patient records are maintained accurately for accessibility.

• Develop and facilitate training sessions for team members on effective therapeutic practices and documentation standards.

• Perform follow-up assessments through interviews and observations to guarantee that the ongoing requirements of each patient are continuously met.

• Serve as a liaison between the mental health community and the general population, promoting the availability of resources to meet developmental goals.

Patient Population / Client Focus\n

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field from an accredited institution.
