Job Summary The Registered Nurse is a professional practitioner who assesses, manages, directs, and provides nursing care activities during the patient’s hospital stay and coordinates planning with other disciplines utilizing a patient/customer driven approach. Job Responsibilities and Qualifications Performs comprehensive nursing assessment/reassessment. Age‑appropriate admission or transfer assessment with family/guardian input. Accurate and complete documentation of findings. Assessment of post‑op/post‑invasive procedure patients. Education and discharge needs assessment of patient and family on admission and throughout hospitalization. Provision of patient reassessment documenting pertinent observations according to plan of care, changes in condition, response to care, procedures, and standards of care. Establishes, coordinates, and evaluates a plan of care based on assessment data, patient diagnosis, lab data, tests, procedures, physician orders, protocols, and standards of care. Identifies short and long‑term goals based on patient care needs. Formulates nursing interventions to achieve desired patient outcome. Incorporates disease‑specific evidence‑based practice into nursing care plan and documentation. Provides and documents nursing interventions based on assessed patient needs, plan of care, and changes in patient status. Collaborates with health team members for coordination of daily plan of care for assigned patients. Provides, coordinates, and communicates patient care, including accurate Handoff Communication Reports. Administers and documents medications accurately according to policies. Monitors, maintains, and documents accurate IV fluids and parenteral nutrition. Completes referrals as indicated by assessment data. Requests consultation for special needs, equipment, or information for patient/family. Provides patient/family education and discharge planning per guidelines and protocol. Clarifies all physician orders as warranted and implements them accurately and timely. Assists physician with procedures/treatments or delegates to Care Partner as appropriate. Documents “Readback” for all telephone or verbal orders. Accepts telephone or verbal orders only in emergency situations. Recognizes changes in patient’s condition and takes appropriate nursing actions. Involves family/guardian in care and decision‑making as appropriate. Recognizes risks for patient and acts appropriately. Incorporates infection control practices into daily care. Documents and/or communicates nursing care and changes in patient condition. Performs and documents ongoing evaluation of effectiveness of care. Evaluates effectiveness of patient/family education. Evaluates and modifies plan of care as indicated. Recognizes significant clinical parameter changes and reports immediately. Identifies problems, gathers pertinent data, suggests solutions, and works toward resolution. Reports variations from care/treatment per reporting policy. Specialized Care for high‑risk patients. Restraint Care: Initiates/evaluates alternatives to restraint prior to application. Applies restraints consistently with approved procedure. Monitors and assesses patient response at appropriate intervals. Provides specified patient care (toileting, skin care, hydration, feeding) timely. Provides consultation for peers on alternatives to restraints and observation. Documents restraint use and associated care thoroughly. Pain Management: Assesses patient for presence of pain. Incorporates patient’s cultural/spiritual beliefs into pain plan. Implements pain techniques; focuses on prevention. Includes patient/family in pain management plan development. Considers other methods of pain control (massage, repositioning, music therapy). Abuse Assessment: Recognizes abuse criteria in assessments. Reports signs of abuse/neglect to appropriate authorities. Takes appropriate action to support patient safety. Demonstrates accountability for professional practice. Adheres to quality, performance standards, policies, procedures, and protocols. Participates in learning experiences that increase competence. Maintains currency in unit information and policies. Attends staff meetings and reviews minutes when absent. Engages in committees, shared governance, and work teams in leadership or membership roles. Reviews nursing publications and updates knowledge. Adapts to changing work demands and supports peers. Becomes a professional resource and supervisor. Assumes and delegates care appropriately. Communicates plan of care clearly to all team members. Supervises and teaches students/new staff nurses. Maintains safety, infection control, and risk management. Disposes of hazardous waste per guidelines. Creates a safe, clean, therapeutic environment. Follows hand hygiene procedures and safe patient handling techniques. Deletes personal conversations and maintains dress code. Reports risk management concerns using eSRM. Completes annual mandatory requirements (licenses, immunizations, trainings). Provides service excellence and customer satisfaction. Delivers prompt, respectful, courteous service. Attends to physical, spiritual, psychological needs of patients and families. Responds promptly to patient requests and communications. Recommends resource‑wise practices and minimizes waste. Maintains confidentiality and effective communication. Incorporates continuous improvement and evidence‑based practice. Assists in nursing unit goal development. Stays current with evidence‑based initiatives. Identifies improvement opportunities, collects data, and proposes solutions. Participates in performance improvement processes and clinical research. Provides age‑appropriate care. Applies knowledge of growth & development for adult and geriatric patients. Assesses and interprets age‑specific data. Uses appropriate communication techniques for age groups. Involves family in decision‑making when applicable. Delivers necessary services to routine age groups served by the department. Job Specifications Minimum Education: Licensed to work as an RN in the state of Missouri. Working Conditions: May be exposed to the risk of blood‑borne diseases; exposure to unpleasant elements (accidents, injuries, illness); subject to varying and unpredictable situations; handles emergency and crisis situations; irregular hours. Physical Requirements: Occasional prolonged standing/walking; occasionally lifts, positions, pushes and/or transfers patients; occasional reaching, stooping, bending, kneeling, crouching. Infection Control Isolate per policy. Adhere to proper infection control practices and educate patients/visitors. Read, learn, follow, enforce and remain up to date on policies and procedures. Practice excellent hand‑washing technique and keep gloves and cleaning supplies ready. Change dressings promptly and maintain invasive lines per policy. Vaccinate per policy. Notify supervisor if risk to patient identified. Use safe practices during crisis (codes) and transport. Nursing Practice Contribute positively to patient care team functioning. Engage in practice development through unit‑based and hospital committees. Ensure thorough, legible documentation of patient care. Round with purpose on each patient. Model practice excellence to new employees. Complete licensure, certifications, and mandatory requirements within deadlines. Environmental Responsibility Maintain proper care of equipment and keep work environment neat. Keep nursing station and break rooms clean and organized. Leave patient rooms in good order with supplies ready for next shift. Fix or report broken items promptly. Give hand‑off reports before leaving unit. People Show empathy to patients and communicate commitment to safety. Respect colleagues, mentor new staff, resolve conflicts constructively. Arrive with a positive attitude. Offer constructive criticism privately and praise publicly. Be part of solutions, not problems. Follow up on patient concerns promptly. Address staff/patient/family needs promptly. Always refer to physicians as Dr. Check patients at appropriate intervals per clinical standards. Service Introduce to patients and families. Explain expected procedures and keep them informed. Respond to call lights promptly. Respect privacy and modesty, manage curtain & traffic during procedures. Explain all procedures, tests, medications, monitoring equipment, and plan of care. Show compassion and empathy to concerns at all times. Maintain alertness and safety in all patient interactions.
