Key Responsibilities\n

• Conduct face-to-face and remote assessments for clients, utilizing evidence-based practices to determine their immediate needs and appropriate interventions.

• Develop and implement integrated care plans that include various health and social services, facilitating access to essential resources for clients.

• Courageously intervene during crises and provide therapeutic support through individual, group, and family therapy sessions.
\n
• Maintain a manageable caseload, ensuring timely monitoring and documentation of clients' progress through regular meetings and follow-ups.
\n
• Participate actively in interdisciplinary team meetings, sharing insights about client progress and adjusting care plans as needed to optimize outcomes.

• Implement safety protocols and conduct screenings to identify risk factors or unsafe situations, escalating concerns when necessary.

• Assist with on-call coverage as assigned, providing critical support during evenings or weekends to ensure client needs are met without interruption.

• Collaborate with community resources to enhance services and improve overall quality of care delivered by the organization.

Your primary focus will involve working with clients in the hospice setting, who are often navigating the challenges of terminal illnesses and associated emotional distress. Qualifications\n

• A valid Licensed Clinical Social Worker (LCSW) license.

• A minimum of two years' experience in a clinical setting, preferably within hospice care, mental health, or related fields.

• A Bachelor’s degree in Social Work or a related field from an accredited institution. A Master’s degree is preferred.

• Proficiency in utilizing electronic health records (EHR) and telehealth platforms to document sessions and monitor client outcomes.

• Exceptional interpersonal skills, with the ability to create a trusting rapport with clients, families, and colleagues.

• Commitment to cultural competence, demonstrating respect and understanding of diverse beliefs and backgrounds.
