We are expanding our Family Medicine team in Granger and are in search of a talented and empathetic Nurse Practitioner to join our close-knit and collaborative group. This role will help us continue delivering high-quality care and meet the needs of our patients with professionalism and commitment.

This role will involve regular travel to the Chicagoland area for training during the initial months. Following the training period, the position will be based in South Bend, Indiana.

Responsibilities

As a Nurse Practitioner with our organization, you will have the opportunity to:

• Conduct patient examinations, document physical assessments, analyze data, create care plans, and treat minor illnesses and injuries according to established protocols. Assist with or perform both invasive and non-invasive procedures within your scope of practice.

• Order laboratory tests, x-rays, and other non-invasive diagnostic procedures. Collaborate with supervising physicians and refer patients to appropriate specialists for issues beyond your scope of practice. Ensure effective communication with referring providers or supervising physicians.

• Prescribe medications within your scope of practice, under the oversight of a collaborating physician, and maintain the necessary mid-level practitioner-controlled substance license.

• Educate patients and their families on health care, illness prevention, and home care, tailoring communication to meet the patient's age, psychosocial, and cultural needs to achieve optimal health outcomes in line with practice protocols.

Qualifications

• A Master's degree from an accredited Nurse Practitioner program and national certification in the relevant specialty (ANCC).

• Current state licensure as a Nurse Practitioner and Registered Nurse.

• Current CPR (BLS) certification.

• An active Mid-level Practitioner Controlled Substance License and DEA License may be required based on prescriptive authority.
