FT Hospice RN Case Manager

Manage hospice nursing cases, primarily admissions Develop, implement, monitor, and update individualized plans of care Communicate and coordinate with physicians, physician extenders, clients, caregivers, client representatives, community health providers, facility staff, and interdisciplinary teams Complete initial and comprehensive physical assessments and obtain illness histories Initiate preventive and rehabilitative nursing procedures Administer medications and treatments as prescribed Provide direct client care under the State Nurse Practice Act Educate and support clients and caregivers on care needs and end-of-life issues Re-evaluate nursing needs and revise care plans as client status changes Coordinate discharge planning Attend interdisciplinary team meetings and ensure care and treatment are communicated and documented Instruct, evaluate, develop plans for, and supervise LPN/LVN and HH/Hospice aides Requirements RN qualification and ability to provide direct client care as defined in the State Nurse Practice Act Ability to complete client, caregiver, and client representative assessments Ability to develop, implement, update, and revise individualized plans of care Ability to administer medications and treatments as prescribed by the physician/physician extender Ability to communicate with physicians/physician extenders and obtain orders as required Ability to educate and mentor clients, caregivers, and client representatives Ability to coordinate care with interdisciplinary teams, community health providers, and facility staff Ability to instruct, evaluate, develop plans for, and supervise LPN/LVN and HH/Hospice aides Ability to attend interdisciplinary team meetings and coordinate discharge planning Availability Monday-Friday, 8:30a-5p, with occasional on-call weekends and typically two holidays per year Core Competencies Demonstrates expertise in managing hospice nursing cases, developing and implementing individualized plans of care, and providing direct client care in accordance with the State Nurse Practice Act. Strong ability to communicate and coordinate with interdisciplinary teams and educate clients and caregivers on care needs.
