Veterans Home of California - Fresno

Opened in October 2013, the VHC-Fresno has 300 beds and presently offers Residential Care Facility for the Elderly (RCFE), also known as assisted living. As a new service, the Home is offering Skilled Nursing. The two levels of care provide continuity in the lives of residents in a homelike atmosphere of dignity and respect. All healthcare needs, meals, transportation and activities are provided by the Veterans Home of California Fresno. Skilled nursing care is provided to residents on a 24-hour basis by licensed nurses and certified nursing assistants. Skilled nursing residents have greater access to rehabilitation therapies, nursing care, pharmacy management, structured activities, and clinical dietary services. A memory care program with this level of care provides a supervised environment for veterans with symptoms of confusion, memory loss, difficulty making decisions, solving problems or participating in conversations. The Veterans Home provides California veterans with a living environment that protects their dignity and contributes to their feeling of self-reliance and self-worth. The nearby City of Fresno offers many cultural and educational opportunities for Home residents to enjoy.

Responsibilities

• Provide for the medically-related social services to assist resident in maintaining or improving their ability to manage their everyday physical, mental, and psychosocial needs.
• Participate as member of the Inter-disciplinary team in developing, implementing, or modifying treatment/care plans, level of care considerations, discharge planning and community referrals, etc. for the resident with the families or responsible parties. Provide counseling or psychotherapy for residents with psychological and/or emotional problems in accordance with established guidelines.
• Provide observation, ongoing assessment, and therapeutic intervention consistent with resident's psychological status. Evaluate effectiveness of resident's treatment and record progress.
• Participate with the Admission Team Process, reviewing applications for prospective residents and conducting Pre-Admission Appraisals.
• Participate in the development of treatment/care plan, initial assessment, level of care considerations, and discharge planning.
• Identify factors, which negatively impact on the resident's physical, mental and psychosocial wellbeing. Assess and refer residents with alcohol related issues. Identify other influential conditions such as depression, family conflict, grief and loss, adjustment, interpersonal problems, chronic or acute pain, and physical or chemical restrains.
• Be aware of resident's rights, identifying the potential for resident abuse, report promptly to Physicians, Administrator or Nursing Supervisor about any adverse behavior toward resident by staff, family member, fiduciary or responsible party.
• Report and document all incident of physical, emotional, sexual, or fiduciary abuse. Assist families with end-of-life planning. Participate as a liaison between Veterans home of Fresno, resident, families, and responsible parties.
• Organize, maintain, and provide necessary health care services consistent with CalVet policies and procedures.
• Prepare, maintain, record and do follow up on resident treatment/care plan with the resident in a timely manner as required by Veterans home of Fresno, and provide oral and/or written reports.
• Provide individual and group therapy for residents in order to achieve identified therapeutic goals.
• Prepare documentation of Social History Evaluations (SHE) and Minimum Date Sheets (MDS) in a timely manner in accordance with Medicare and Title-22 regulations.
• Act as an advocate for residents to ensure appropriate treatment is provided and grievance policies and procedures are followed. Assist with Advance Directives, Power of Attorney and Assistance Payee service's paperwork.
• Given the circumstance and environment all duties/responsibilities assigned to the contractor/provider are subject to change and modification.

Requirements

• Must have a Master's Degree in Social Work from an accredited University.
• Must have a clear, full, unrestricted LCSW license in California.
• Clinical Competencies must be current.
• Must have Professional Liability Insurance.
• COVID Vaccine Record or medical religious exemption.
• 2 Professional References.
• Must have CPR certification (BLS through the American Heart Association or Red Cross).
