Job Overview
We are seeking a detail-oriented, knowledgeable, and motivated Licensed Practical Nurse (LPN), specializing in Home Care with a focus on compliance and chart review. This role involves ensuring accurate medical documentation, adherence to CMS (Centers for Medicare & Medicaid Services) Standards, and maintaining high-quality patient care through meticulous review processes. The ideal candidate will possess strong clinical knowledge, familiarity with Home Health coding, and experience in utilization review within Home Health settings.

Duties

• Review and analyze OASIS (Outcome and Assessment Information Set) data for accuracy, completeness, and compliance with regulatory standards.
• Conduct thorough audits of medical records to ensure documentation aligns with Medicare requirements and NCQA (National Committee for Quality Assurance) standards.
• Utilize EMR (Electronic Medical Record) systems such as HCHB (Home Care Home Base) to extract, review, and document clinical data efficiently.
• Collaborate with, educate, and train clinical staff to verify proper documentation of patient assessments and treatments.
• Apply knowledge of coding classifications for accurate medical management documentation.
• Support compliance initiatives by identifying documentation deficiencies and recommending corrective actions.

Qualifications

• Valid licensure as a Licensed Practical Nurse with current state certification.
• Strong knowledge of Medicaid/Medicare regulations and guidelines
• Proven experience in Certified Home Health Agency (CHHA), and nursing environments.
• Familiarity with EMR/EHR systems, including HCHB.
• Excellent attention to detail with the ability to review complex medical records accurately while maintaining confidentiality under HIPAA guidelines.
