Therapist - Physical Therapist

$2720.00/week –

Job Summary:
The Physical Therapist is responsible for evaluating and treating patients to improve and restore functional mobility following illness or injury. They develop personalized therapy plans to help clients achieve optimal performance within their capabilities and promote overall well-being.

Responsibilities:

• Assess patients’ physical condition and evaluate their treatment needs.

• Develop and implement customized physical therapy programs based on physician prescriptions.

• Monitor patient progress and adjust treatment plans accordingly.

• Document clinical observations, treatment interventions, goals, and discharge plans accurately.

• Supervise Physical Therapy Assistants and collaborate with multidisciplinary teams as needed.

• Perform initial admission/evaluation visits and communicate OASIS scores to the Clinical Team Manager within designated timelines.

Qualifications:

• Hold an active state licensure as a Physical Therapist.

• Possess at least one year of experience in Home Health therapy.

• Demonstrate strong clinical assessment and treatment planning skills.

• Exhibit excellent communication and documentation abilities.

• Maintain CPR and BLS certification.

• Show the ability to work independently and as part of a healthcare team.
