Case Planner II (FHS-EFFC)

As a Foster Care Case Planner , you will play a critical role in helping children and families navigate challenges, build on their strengths, and achieve lasting permanency through reunification, adoption, KinGAP, or successful independent living. Working alongside foster parents, biological families, community partners, and multidisciplinary teams, you will serve as an advocate, coach, and change agent for some of New York's most vulnerable children. Summary of Job Duties Champion Permanency for Children Partner with children, youth, families, and caregivers to develop and implement individualized permanency plans. Support successful reunification efforts whenever safely possible. Engage foster families in concurrent planning and support pathways to adoption or KinGAP when appropriate. Help older youth prepare for successful independent living and adulthood. Strengthen Families Assess safety, risk, strengths, protective factors, and family needs. Collaborate with families to develop safety plans and service plans that promote long-term stability. Provide guidance, support, and coaching to parents and caregivers to strengthen parenting skills and family functioning. Partner with parent advocates to help families overcome barriers and achieve reunification goals. Advocate for Children's Well-Being Coordinate services with schools, medical providers, mental health professionals, community organizations, and support networks. Conduct home visits with parents, foster parents, and discharge resources. Facilitate and supervise family and sibling visitation as required by the service plan. Support youth in developing life skills, healthy relationships, and future planning. Lead Case Planning and Documentation Complete psychosocial assessments, FASPs, court reports, adoption home studies, kinship home studies, permanency reports, and other required documentation. Attend Family Team Conferences, court proceedings, permanency hearings, and case reviews. Maintain timely and accurate progress notes, service plans, and case records in compliance with agency and regulatory requirements. Prepare adoption summaries and support adoption planning activities. Provide Crisis Support and Care Coordination Respond to crises and provide intervention when needed. Assist with hospital admissions, discharge planning, and coordination of aftercare services. Coordinate with sociotherapists and service providers to monitor progress and ensure service effectiveness. Advocate for children and families across multiple systems, including education, behavioral health, medical, and community-based services. Be Part of a Collaborative Team Participate in weekly supervision, team meetings, consultations, and professional development activities. Implement evidence-based practices and interventions with youth, families, and caregivers. Contribute to a culture of learning, innovation, and continuous improvement. Qualifications Master's degree in Social Work, Counseling, Psychology, Human Services, or a related field. Ability to engage children, youth, and families in a respectful, strengths-based manner. Strong assessment, documentation, organizational, and case management skills. Excellent interpersonal, communication, and relationship-building abilities. Proficiency with computers and case documentation systems. Valid driver's license preferred. Must have the ability to travel throughout the community via public transportation. Should have experience working in foster care, child welfare, family services, behavioral health, or a related field. Experience working with families affected by substance use, trauma, mental health challenges, or system involvement is preferred. Bilingual Spanish/English highly preferred.
