Licensed Practical Nurse -Home Care Weekends

Position Details City/State: Virginia Beach, VA Work Shift: Weekend Days Position Status: Full Time, Day Shift, Weekends Location: Virginia Beach, VA Pay Range: $22.39 - $37 per hour Mileage reimbursement is offered for all miles driven between patient visits. Overview Sentara Home Care Services is seeking a qualified Licensed Practical Nurse (LPN) to join our team. The LPN works under the supervision of an RN or Licensed Independent Practitioner (LIP) and is responsible for delivering direct patient care in a variety of settings including hospitals, nursing homes, assisted living facilities, outpatient clinics, or private homes. Responsibilities Provide direct patient care, including assessment, testing, and medication administration. Implement nursing interventions and care plans to support a holistic approach to patient care. Serve as a patient advocate in the prevention of illness or disease. Collaborate effectively with physicians, NP, PA, and other multidisciplinary team members. Communicate and educate patients and family members regarding care and treatment. Document patient information accurately and maintain compliance with legal and ethical standards. Qualifications Education: Trade School Graduate (Required). Certification/Licensure: Active LPN license in Virginia. BLS certification required within 90 days of hire. Minimum 2 years of clinical experience (required).
