Alternate Solutions Health Network is seeking an RN Case Manager to lead patient assessment, plan of care and coordination of multidisciplinary teams in home health. You’ll educate patients and families, ensure safety, complete documentation, and stay compliant with regulations under physician supervision. This role requires strong clinical judgment and excellent communication in a fast-paced setting.
