Responsibilities
- Perform comprehensive physical therapy evaluations for residents in a skilled care setting
- Develop and implement individualized treatment plans focusing on mobility, gait, balance, strength, and range of motion
- Monitor progress, document outcomes, and adjust plans as needed
- Provide gait and transfer training, fall prevention strategies, and ROM exercises
- Educate residents and families on home exercise programs and safety measures
- Collaborate with an interdisciplinary team and participate in care planning discussions

Requirements
- Active Physical Therapist license or eligibility for licensure in the appropriate state
- Doctor of Physical Therapy (DPT) degree or equivalent
- Experience or demonstrated interest in geriatric rehabilitation or skilled nursing facility settings
- Proficiency with electronic medical records and clinical documentation
- Ability to work effectively in a skilled nursing environment and follow established care plans
- Strong communication and teamwork skills
