Collect data on residents’ physical and psychosocial status and communicate findings to the RN and Interdisciplinary Team. Contribute to development of an age-appropriate, culturally relevant care plan reflecting nursing diagnoses and interventions; involve resident and family in the care planning process. Recommend revisions to the plan of care based on outcome evaluation and coordinate referrals to ensure comprehensive care. Maintain current nursing knowledge and competency; participate in mentoring or orientation of staff; demonstrate leadership within the professional practice setting. Maintain safety and infection control standards; handle situations with potential exposure to blood or body fluids in accordance with policy.
