Licensed Practical Nurse

Schedule: 6:30 PM - 6:00 AM, Full Time or PRN available. Block scheduling which rotates. Always 2 days off in a row and every other weekend. Administer medication and perform treatments as ordered by the physician and within scope of license. Medications may be in a variety of forms including oral, enteral, parenteral, and suppository. Treatments may include sterile and aseptic wound cleaning and dressing, postural drainage and percussion, tracheostomy, gastrostomy and jejunostomy care, and catheter care. Observe and report the effectiveness of medications and treatments. Ensure an adequate amount of supplies and medication are available. Receive and deliver a variety of information regarding residents' medical, social, functional, and other needs. Participate in the receiving and giving of shift reports. Communicate with residents, families, and staff as necessary to ensure appropriate care from the interdisciplinary team. Complete the initial admission nursing assessment tool on newly admitted residents. Assist with the development of Minimum Data Set assessments, ongoing quarterly reviews, and care plans. Maintain appropriate clinical and administrative documentation regarding diagnosis, observations, treatment, and summary of resident progress. Participate in Quality Assurance and Performance Improvement programs. Attend and participate in staff meetings. Knowledge of gerontological/geriatric nursing theory and practice. Understand the effects of the aging process. Knowledge of practices and procedures of team nursing. Familiarity with practices and procedures for implementing treatment plans for resident/patient care. Knowledge of practices and procedures of pharmacology and medicines. Awareness of federal, state, and agency laws, rules, and regulations governing general practical nursing practices. Skills in observing and reporting behavior and progress of residents/patients. Ability to apply and interpret state and agency laws and regulations governing geriatric nursing practices. Assist in the development and implementation of patient care plans within a team nursing concept. Strong oral and written communication skills. Interpersonal skills are essential. Ability to abide by nursing regulations. Listen, observe, assess and respond to medical emergencies. Evaluate, analyze, make independent decisions, and follow up regarding client health problems. Respond to medical issues/inquiries in a professional manner. Establish and maintain effective work relationships.
