Job Summary

The Licensed Practical Nurse (LPN) delivers high-quality, direct patient care under the general supervision of a Registered Nurse (RN). This role serves as a primary clinical channel of communication between the patient and the care team, executing treatments, monitoring outcomes, and ensuring strict adherence to infection control policies.

Key Responsibilities

Provide comprehensive direct nursing care , including managing personal hygiene and assisting in formulating and updating patient-centered care plans.
Administer prescribed medications and clinical treatments , including respiratory therapy modalities, maintaining meticulous healthcare logs.
Observe, evaluate, and rapidly report shifts in patient health conditions or emergency indicators to the supervising RN.
Instruct and educate patients and their families during hospitalization stays and throughout the discharge protocol.
Document clinical assessments and baseline activities using standard electronic medical records (PointClickCare).

Mandatory Requirements

Education: Graduate of an approved, accredited Licensed Practical Nurse program.
Licensure: Active, unrestricted Practical Nurse (LPN) license.
Certifications: Active Basic Life Support (CPR) and Advanced Cardiovascular Life Support (ACLS) certifications.
Technical Skills: Competency in computer documentation applications and clinical charting.
